Border Osteo & Performance is an osteopathy and strength and conditioning practice at Club Forma Gym, 71–75 Murphy St, Richmond VIC 3121. We follow the Australian Privacy Principles under the Privacy Act 1988 (Cth).
What we collect
Contact details, such as your name, mobile number and email, when you book, contact us or fill in a form, including forms on Facebook or Instagram.
Health information you give us as part of your care, such as your history, assessment findings and treatment notes.
Payment and health fund details needed to process appointments.
Website data, such as pages visited, collected through cookies and tools like Google Analytics and the Meta Pixel.

We will only use your information for these purposes, or as the law allows.
How we store it
Your records are kept securely in Cliniko, our practice management system, which stores data in Australia. Enquiries made through Meta lead forms are kept in Meta’s systems until we move them into our records.
Who we share it with
We don’t sell your information. We only share it:
with other health practitioners when you agree, for example in a referral
with your health fund, to process claims
when required or allowed by law

You can ask to see or correct the information we hold about you at any time.
